Health History Form
To insure the best care possible, all students entering Westmont are required to have a health history form filled out before being seen/treated in the Health Center.
 
Physical Exam
All students entering Westmont College are highly recommended to have a physical exam before coming to campus. Print this form to take with you to your doctor's appt to be completed. Be sure to get a copy of your immunization records, if applicable, from your physician's office. See below for notations regarding immunizations.

Consent Form
All students entering Westmont College who are under 18 years of age must have a consent form signed by a parent/guardian to receive treatment in the Health Center.
 
To determine the need for a TB test, please complete the TB Questionnaire:

The requirements for TB testing has changed. Please review and complete this questionnaire. If necessary, print the TB Risk Assessment form and bring to your physical exam for your doctor to complete.
 

If you have answered yes to any of the questions, print the following form and bring with you to your doctor's appointment for your physical.  TB Risk Assessment

Once you have all your forms printed and completed, you can mail it to the Health Center along with your Immunization Records* and a copy of your Insurance Card to the following address:

* We are not requiring up to date vaccinations in order to attend Westmont-- however we strongly back CDC's recommendations to get them. Your primary care provider will be able to get you on a schedule to begin getting vaccinations, if that's what you choose to do.
